Problemas con PHP porfavor ayudenme.

Recibo un menaje de error en un ejemplo basico tengo IIS 6.0 PHP 4.0 y MySQL 4, y cuano trato e ingresar un registro a la base de datos me dice lo siguiente:
Notice: Undefined variable: nombre in C:InetpubwwwrootAvisos Iquiqueagregar.php on line 4

Porfavor si alguien tiene o sabe alguna solucion.

debes estar usando una funcion header(); esta se debe invocar antes de devolver ningun html es decir antes del echo '<html>'

lo que pasa es que no has definido la variable nombre en la linea 4 de tu script... recuerda que toda variable lleva antes $
$tuvariable = "pon algo";
salu2
$tuvariable = "pon algo";
salu2
